Each monthly promotion (“Promotion”) is promoted and sponsored by Leslie’s Poolmart, Inc., 2005 E. Indian School Rd., Phoenix, AZ 85016 (“Leslie’s”). No purchase of any kind is necessary to enter to win. Making a purchase will not increase your chances of winning. Void where prohibited by law.

Eligibility No purchase of any kind is necessary to enter to win. Each Promotion is open to all legal residents in the United States who are at least 18 years old at time of entry. Each Promotion will continue on a monthly basis until terminated by Leslie’s. Employees, owners, officers, directors and their immediate family members (spouse, parents, siblings, and children) of employees of Leslie’s (d/b/a “Leslie’s Swimming Pool Supplies”) and each of its respective parents, subsidiaries, affiliates, directors, owners, employees and agents are not eligible to participate.

Monthly Promotion Period The entry period for each specific month’s Promotion begins at 1 a.m. Eastern Time (“ET”) the first day of that month and ends as of 11:59 p.m. ET on the last day of that month (“Promotion Period”). Winners will be selected from eligible entries properly submitted during each Promotion Period.

Entry Instructions To participate in any Promotion there are two ways to enter the Promotion: (1) online entry via the electronic survey or (2) alternate method of entry. Online Entry Via the Electronic Survey: (1) Store visit satisfaction survey: visit the website www.lesliespool.com/opinion Home service visit satisfaction survey: visit the website http://lesliespool.com/tellservice Online or phone order satisfaction survey: visit the website http://lesliespool.com/myorder (2) when prompted fill-in the requested information found on the store sales receipt, survey invitation email (online or phone order) or the call number from the onsite service work order, then complete the electronic survey in its entirety, and (3) provide a valid email address (for prize notification). This fully completed survey will act as the participant’s entry in a Promotion. Said entries must be received by Leslie’s during a Promotion Period to be eligible for that month’s Promotion. Alternative Method of Entry: Alternatively, you may also enter by mailing a letter or postcard with your first and last name, exact street address and valid email address to Leslie’s Customer Satisfaction Survey Promotion, 2005 E. Indian School Road, Phoenix, AZ 85016, attention Marketing Department. Alternative entries must be postmarked and received during a specific Promotion Period to be included in the random drawing for that Promotion Period. Failure to comply with this or any other official rule will result in disqualification. Leslie’s is not responsible for inaccurate entry information or for lost, misdirected, late, incomplete, illegible or unintelligible entries, whether caused by participant or by any of the equipment or programming associated with or utilized in this promotion, or by any technical or human error that may occur in the processing of entries. Mechanically reproduced entries are not permitted and will be deemed void. There is a maximum of one entry per transaction or letter per day. Illegible/incomplete entries are void. All entries become the property of Leslie’s and will not be acknowledged or returned.

Prizes For each Promotion, one (1) winner will receive $500 in Leslie’s Customer Appreciation Dollars (“Prize”) that can be used at any Leslie’s retail location. To redeem for an online purchase, winner must call Leslie’s Customer Service at 1-800-537-5437. Each Prize may only be used exclusively for the purchase of pool and spa supplies at Leslie’s Swimming Pool Supplies retail stores or ecommerce website (www.lesliespool.com)). The Prize will expire thirty (30) days after notification is sent by Leslie’s to the winner. By participating in a Promotion, entrants waive the right to claim any additional damages whatsoever, including, but not limited to, punitive consequential, incidental or indirect damages.

Approximate Prize Value The total retail value of each month’s Prize is approximately five hundred United States dollars ($500).

Prize Drawings Approximately 5 business days after the end of each month’s Promotion Period, there will be one (1) random drawing from all eligible entries received during that Promotion Period; such drawing shall be final and binding with respect to all matters related to that Promotion. Winners need not be present to win. Winners will be notified via the email address provided and will have three (3) business days to respond. Failure to respond within the three (3) day period shall result in forfeiture of the Prize by the winner. No alternative winner will be drawn for any Promotion Period. Entrants without a proper email address will be excluded from the drawing. Winner(s) have thirty (30) days from email notification to claim the Prize. No Prize substitution, cash equivalent or transfer of the Prize by winner in violation of these rules is permitted. Leslie’s reserves the right to substitute a prize of equal or greater value. Promotion Prizes are subject to all applicable federal, state, and local laws, rules and regulations. All federal, state and local taxes are the winner’s sole responsibility. Prize winners may be asked to present a valid driver’s license or other form of identification at time of prize award for age verification. Failure to complete or noncompliance with the Affidavit of Eligibility and Release requirements may result in disqualification effective immediately. Prize winners consent (and, except where prohibited, agree to sign any additional documents required by Leslie’s to formalize, effect of perfect such consent) to Leslie’s use of his/her name, likeness (photograph and/or video), biographical information, and voices in advertising/publicity/trade without compensation, notice or approval; and Prize winners disclaim any ownership rights of such advertising/publicity/trade material (TN residents will not be required to sign a publicity release as a condition of winning a prize). Should a Prize winner make any false statement in connection with this Promotion, the winner will be required to promptly return to Leslie’s, upon demand, the prize. All prizes are awarded “as is”, and without warranty of any kind, whether express or implied.

Odds of Winning Odds of winning a prize depend on the number of eligible entries received in the monthly drawing. Limit one prize per entrant per 12 (twelve) month period.

Taxes and Expenses Winners are solely responsible for any local, state and federal taxes. A 1099 tax form may be issued to a winner in connection with the Prize. Any additional expenses incidental to any Prize are the responsibility of each respective winner.

Additional Terms Leslie’s reserves the right, in its sole discretion, to modify, terminate, or suspend any Promotion should viruses, bugs, unauthorized human intervention or other causes beyond Leslie’s control corrupt or impair the administration, security or fairness of the Promotion. Leslie’s is not responsible for any human error that may occur in the processing of the entries, nor is it responsible for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries, whether caused by the sender or any of the equipment or programming associated with or utilized in any Promotion. Leslie’s reserves the right, in its sole discretion, to disqualify any individual found to have tampered with the entry process or the operation of the promotion; to have acted in violation of these rules; or to have acted in an unsportsmanlike or disruptive manner or with intent to annoy, abuse, threaten or harass any other person. Any use of robotic, automatic, programmed or similar entry methods will void all such entries submitted by such methods. Leslie’s and its agents are not responsible for any printing or technical error which may occur in processing, printing or distribution of promotion materials.

Miscellaneous By participating in a Promotion, participants agree to be bound by these rules. No transfer, assignment, cash redemption or substitution of prizes is permitted. By participating in a Promotion, participants agree to release and hold harmless Leslie’s, and its respective parents, subsidiaries, affiliates, directors, owners, employees and agents (collectively, the “Released Parties”), from and against any claim or cause of action arising out of participation in the Promotion or the receipt, redemption or use of any prize, even if caused or contributed to by the negligence of Leslie’s, and its respective parents, subsidiaries, affiliates, directors, owners, employees and agents. Participants agree that the decisions of Leslie’s related to any Promotion and all drawings are final. All personal information collected through any Promotion is subject to Leslie’s Privacy Policy, located at www.lesliespool.com/privacyandsecuritypolicies.htm. By entering any Promotion, you agree to waive any right to claim ambiguity or error in these Official Rules or in the promotion itself.

Limitation of Liability The Released Parties do not have any responsibility or liability whatsoever for: (a); unauthorized human intervention in any part of the entry process or the Promotion; (b) electronic or human error which may occur in the administration of the Promotion or the processing of entries; (c) any property loss, damage, personal injury, or death, in connection with the prizes provided pursuant to the Promotion and/or any person’s participating in the Promotion; (d) any printing or typographical errors in any Promotion-related materials; (e) stolen, lost, late, misdirected, corrupted, incomplete, damaged or indecipherable entries; or (f) technical failures of any kind, including but not limited to the malfunctioning of any computer, mobile device, cable, network, hardware or software (if applicable), as well as any failure of the website during the Promotion Period. Under no circumstances will participants be permitted to obtain awards for, and participants hereby waive all rights to claim, punitive, incidental or consequential damages, or any other damages, including attorneys’ fees, and participants further waive all rights to have damages multiplied or increased. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of participants and Leslie’s in connection with these promotions, shall be governed by, and construed in accordance with, the substantive laws of the State of Arizona, without regard to Arizona conflict of law principles.
